Aim The adequate dosing of topiramate in neonates undergoing therapeutic hypothermia has not been established. The aim of this study was to design a dosing schedule capable of providing topiramate serum concentrations within the accepted therapeutic range. Methods Neonates (n = 52) with hypoxic ischaemic encephalopathy and subjected to therapeutic hypothermia were dosed with topiramate, 5 mg/kg on day one and 3 mg/kg on days two to five, to decrease seizure events. Serratia marcescens infection or hypoxic-ischaemic encephalopathy in neonates: Is magnetic resonance imaging a problem-solving tool?

To the Editor: We read with great interest the retrospective case series by A Madide and J Smith,[1] describing brain ultrasound (US) findings of neonates with Serratia marcescens hospital-acquired infections. S. marcescens is a Gram-negative organism that frequently colonises neonatal intensive care units (NICUs). This bacterium can cause severe brain infections in neonates, with irreversible neurological damage and long-term neurodevelopmental impairment. Abstract Introduction Hypoxic-ischaemic encephalopathy is one of the main causes of neurological damage in the new-born. Therapeutic hypothermia is the current treatment to reduce mortality and disability in new-borns with this condition. Objective To identify nursing care in new-borns with severe to moderate EHI, treated with active therapeutic hypothermia.
